HEAD AND NECK SURGICAL ONCOLOGY CONSULTATION
Flexible laryngoscopy: exophytic lesion of the left base of tongue crossing into the vallecula and abutting the epiglottis; airway patent.
Assessment: recurrent rT2 N0 SCC in a previously irradiated field.
Discussion: Salvage resection would require total glossectomy with total laryngectomy and free flap reconstruction to obtain clear margins. The patient declines salvage surgery after a detailed discussion of functional outcomes.C-11 Referred to radiation oncology for re-irradiation.
