1. Referral
a. An approved VA community care referral with a standardized episode of care (SEOC) covering proton therapy is on file.C-1
2. Clinical documentation
a. Pathology confirming the diagnosis is included.C-2
b. Staging, and for prostate cancer the risk group, is documented.C-3
3. Dosimetry
a. A comparative photon plan is included when requested by the VA radiation oncology reviewer.C-4
4. Plan of care
a. The community provider submits a plan of care signed by the treating radiation oncologist.C-5
